On this page we have all of the forms that you need to fill out before your first visit at Mesquite Pediatrics.  If you have a newborn, you only need to fill out the first four forms.  If you have an older child, we would like to get records from your previous pediatrician before scheduling an appointment, unless it is an emergency.  Please fill out the top four forms for us and also fill out the release of records form and send it to your previous physician.

On the other hand, if you are leaving Mesquite Pediatrics and would like your records transferred from us to a new pediatrician, fill out the release of records form below and send it to us.  We will then send a copy of your records to the new physician.
