We have the Moderna COVID-19 vacccine and will have drive-through vaccine clinics for people 18 years and older by appointment. BEFORE coming to your appointment, you MUST print out and complete the first page of the Prevaccination Checklist for COVID-19 Vaccines and the ADHS COVID-19 Vaccine Consent Form and bring them with you to your appointment. If you do not bring these forms, your vaccine may be delayed. Click the links below to download the forms.
- COVID-19 Vaccine Screening Questionnaire - Your answer must be "No" to questions 1, 2, 3, and 6. If your answer is "Yes" to any of these questions, please do not schedule an appointment. If you answer "Yes" to any of the other questions, you can probably still get the vaccine. There is information on subsequent pages of the questionnaire that discusses these circumstances and we will make a determination on a case by case basis.

Unformtunately we have no vaccine appointments available at this time. Please follow our Twitter feed to be notified when we open up more availability or when we have extra doses to give on a same day basis.

- V-Safe Information - information about a vaccine side effect tracking program run by the CDC to help increase our knowledge of side effects of the vaccine. Enrollment in the program is recommended but not required.
